When the primary rehabilitation process or even step-down programme concludes, some individuals may find themselves not fully ready to reintegrate into society. They may opt for a transitional care recovery journey. Upon completing this process, healthcare professionals often recommend transitioning into a rehabilitation phase to reintegrate into society. During this period, individuals in recovery return to normal life with their families but continue to receive personalised consultations with a physician weekly. During their enrollment in the Transition Program, participants will receive

  • Daily Life: Participants can engage in work, activities, and lead a normal life within the community.
